• Copies of discharge documents (DD-215/DD-215) - if registered with the State of Wisconsin or our office
• Wisconsin Driver's License - Veterans Identifier (Refer to the Veteran and DoD ID Cards section)
• Wisconsin State Park Pass - for Veterans with a 70% or higher service-connected disability
• "Save the Date" known as an Intent to File (VA Form 21-0966) Hint: Easier to call 1-800-698-2411
• Client Representative Form  (VA Form 21-22)
