Community Health Assessment (CHA) & Improvement Plan (CHIP)

What is a Community Health Assessment?

A Community Health Assessment (CHA) is a collaborative, community-centered process to collect, analyze, and share data on the health of the community. As a Public Health Accreditation Board (PHAB)-accredited health department, Outagamie County Public Health (OCPH) is required to engage community organizations in the CHA process.

We collect primary data, secondary data, and demographics for the population we serve. The OCPH CHA(PDF, 8MB) was last released in December 2021.

What is a Community Health Improvement Plan?

A Community Health Improvement Plan (CHIP) is a local health plan to address health conditions that affect residents based on data from the Community Health Assessment. The Outagamie County Public Health (OCPH) CHIP process focuses on cross-sector collaboration with community partners, alignment with national and state priorities on social determinants of health, and a focus on equity.

The OCPH CHIP 2023-2025(PDF, 16MB) was released in March 2023. t is a living document. Our team reviews and updates the CHIP every year to continue to provide opportunities for health where people live, learn, work, and play.

Why do we have a CHA and CHIP?

Local health departments throughout Wisconsin must develop a CHA and CHIP every five years. For over 30 years it has been required through Wisconsin State Statutes. A review also takes place every year on the progress on objectives, strategies, and measures.

Community Health Improvement Plan 2023-2025

We are excited to release our 2023-2025 Community Health Improvement Plan (CHIP)(PDF, 16MB).

Through collaboration with more than 40 community organizations and collection of over 300 community votes, we identified three key health priorities:

  • Mental Health
  • Access to Care
  • Housing and Homelessness

In addition to the Community Health Improvement Plan, OCPH created a CHIP Supplemental Document(PDF, 4MB) outlining the process of how we engaged the community to develop the plan.

Also, during the CHIP process, our team publishes annual reports to update community partners and stakeholders of the progress on work being done. The 2025 CHIP Annual Update(PDF, 3MB) is now ready for you to view.
