Requesting Copies of Vital Records
The Register of Deeds office maintains and issues: birth, death, marriage, divorce, and domestic partnership records. Certified copies require a direct and tangible interest as defined on the application.
STATEWIDE ISSUANCE OF VITAL RECORDS
If the vital record you are requesting occurred in the State of Wisconsin and falls within the dates listed below, you can obtain the copies from ANY Register of Deeds office in the State of Wisconsin.
Birth Certificates - 10/1/1907 to present
Death Certificates - 9/1/2013 to present
Marriage Certificates - 10/1/1907 to present
Divorce Certificates - 1/1/2016 to present
Request a vital record in person
Visit our office to purchase a vital record. Applications are available at our service counter or you may print one online (see Application Forms).
The fee for a vital record is $20.00 for the first copy and $3.00 for each additional copy.
We accept credit/debit cards (additional fees apply) or cash. Checks are not accepted at the counter.
Identification showing your name and address must be provided with all applications.
- Expired cards or documents are not acceptable.
- A valid street address, not a P.O. Box, is required for all counter applications.
Acceptable forms of ID include:
ONE of the items from this list:
- State-issued driver's license or ID card
- U.S. government-issued photo ID
- U.S. or foreign passport
- Tribal or military ID card
Or TWO of the items from this list:
- Bank/earnings statement
- Current, dated, signed Lease
- Health insurance card
- Utility bill or traffic ticket
- Vehicle registration/Title
Request a vital record through US Mail
Complete and print the application form (see Application Forms). Submit with a photocopy of acceptable identification and correct payment.
Cost of a vital record is $20.00 for the first copy of each record, and $3.00 for each additional copy of the same record.
We accept cash, money order, or personal check; however, all check payments are subject to a three (3) day waiting period before mailing out the certificate(s).
