Your WIC Appointment

Before Your Appointment

  • You will get a link to complete intake and sign forms one week before your appointment.
    • You will get a reminder the week of your appointment.
      • If you have not completed intake we will resend the intake the week before.
    • WIC staff will call to remind you of the appointment the day before.
      • If intake is not completed we will resend the day of the appointment.
  • Phone appointments: Please make sure intake is completed and forms are signed, if needed. WIC staff will call you around the appointment time. If you did not complete intake or sign the WIC Rights and Responsibilities WIC staff will not call as we are unable to complete the appointment.
  • If you miss your appointment please call as soon as possible so we can get you in at our next available appointment time.

What happens at my appointment?

You may need to provide this information to us:

  • Address and income for the past 30 days for your household.
  • Proof of ID for you and your children getting WIC benefits.
  • See the Wisconsin WIC webpage for examples of proof of identity, address, and income.
  • If you receive Medicaid or FoodShare we may be able to access verification information for your appointment. WIC Staff will send the intake for what is needed to complete the appointment.

We’ll do a screening of you and/or your child's health and nutrition. If you have an in-person appointment, this will include:

  • Height and weight measurements
  • Questions about your health and diet
  • A test to see how much iron is in your blood

You’ll get an eWIC card at your appointment that you’ll be able to use that day to buy food.

Important Information about Your Appointment

  • Please arrive 15 minutes early for your appointment.
  • If you or your child are sick, please call us at (920) 832-5109 to reschedule.
  • Intake will be sent with the information we need you to submit for your appointment.
