Looking for trustworthy, approachable health information in a welcoming community setting? Everyone is welcome at this free series presented with Grapevine. Through thoughtful presentations and expert guidance, this health information series offers helpful resources and practical information to support healthier us to live a little better every day. Hosted by Kaukauna Public Library staff member Bethany and nurses Emilie and Vicki of the Outagamie County Public Health Division in ABC. Registration opens two weeks before each class to allow time for supplies and preparation. 

Advance care planning is a process of understanding, reflecting on, and discussion future medical decisions regardless of age and current health status. All adults (18 and older) should think about, discuss, and create an advance care plan. Having an advance care plan is truly a gift for yourself and others. Note: Advance Directive forms will be provided but NOT be completed during the session. Information on where and how to complete this document will be covered during discussion.
