Applications should be filled out and returned to the Child Support Agency. If you have an existing child support case, return applications to the county where your case is active.

Complete these forms when you are court ordered to seek work. Whether it be a part of your regular court order or from being enrolled in the Children First Program.

If you feel the other party is not abiding by the court order and wish for them to be found in contempt, fill out the Affidavit for Contempt, and take the paperwork to the FCC (located in the main building) to obtain a court date. They will fill out the OTSC and will instruct you on how the other party is to be notified.

Affidavit for Contempt

To change Custody/Placement/Support/Maintenance/Arrears, fill out the Affidavit and the top portion of OTSC. Take the paperwork to the FCC (Located in the main county building) to obtain a hearing date/time. They will then tell you how to go about notifying the other party.

Affidavit for Modification

To provide the court with a comprehensive disclosure of their financial situation (sometimes required when filing a motion):

If parties are in agreement to change Legal Custody, Placement, Child Support, Maintenance, Family Support, or Arrears a “Stipulation to Change” and “Order on Stipulation to Change” can be filled out. Once filled out and signed by both parties and the Child Support Agency (GAL as well if appointed). The order then needs to go to Clerk of Courts to be filed and stamped. A copy of the file stamped order should then be returned to the Child Support Agency downtown Appleton office.

Stipulation to Amend the Pay Record: If parties are in agreement to modify the debts owed, this form will need to be filled out and signed by both parties and The Child Support Agency and then taken to FCC (located in the main county building) to be approved before being filed with Clerk of Courts. A copy should be returned to the Child Support Agency once it’s filed and stamped.

Child support payments that you receive can be deposited directly into your checking account. Fill out the form and mail it back to the trust fund with either a voided check or an Authorization Letter, identifying you and your account, from your financial institution. Do not return this form to the Child Support Agency, it needs to be returned to WI SCTF.
