Nutrition Program

The purpose of the Nutrition Program is to reduce hunger and food insecurity, promote socialization and the health and well-being of persons aged 60 or older by providing assistance in gaining access to nutrition, disease prevention and health promotion services to delay the onset of adverse health conditions resulting from poor nutritional health or sedentary behavior. The Nutrition Program provides Congregate and Home Delivered Meal Service along with nutrition education, screening and counseling in accordance with the Federal Older Americans Act.

Nutrition Program Dining Options for Older Adults

What is Congregate Meal Service? Congregate Meal Service is dining in a communal setting for individuals 60 years and older; their spouse, regardless of spouse’s age; and/or disabled adult, under 60, residing with an eligible older individual and/or reside in a housing facility that is occupied primarily by older individuals, at which congregate nutrition services are provided.

Congregate meals offer opportunities for socialization, nutrition education and counseling, a link to the ADRC, and other resources used to support the well-being and independence of older adults.

When do I sign up? You must call and make a meal reservation before 12:00 NOON the day before eating at a Congregate Mealsite! Participants who make a meal reservation in advance will be given priority. A participant registration form is required annually.

Who do I contact to sign up? You will speak with the Mealsite Manager to reserve a meal. If you would like to make a meal reservation, please contact the Mealsite 24 hours in advance according to the chart below:

Contributions- Vital Support for the Nutrition Program

Contributions, A Vital Support for the Nutrition Program

The Federal Older Americans Act states that any person 60 years or older is eligible for Nutrition Program meal service regardless of income.  No fee may be charged for meal service.  The Contribution Policy for the Outagamie County Nutrition Program states that eligible participants must be given the opportunity to make a voluntary contribution toward the cost of the meal service.  Participants must decide for themselves how much they are able to contribute.   Participants who are unable to contribute the full meal cost may not be denied meal service.  All Outagamie County Nutrition Program contributions will remain confidential.

Participant contributions are important to the continuation of the Outagamie County Nutrition Program.  Participants may contribute with the knowledge and understanding that their contribution is being used to support and strengthen the future of the Outagamie County Nutrition Program.

Emergency and Supplemental Foods

Nutrition Program Emergency and Supplemental Foods

PLAN AHEAD! Have the following foods available in the event of severe weather or hazardous conditions.  These nutritional foods may also enhance the Nutrition Program meals you receive:

  • Bottled water
  • Quick cooking or instant hot cereals
  • Prepared cereals (whole grain, enriched preferred)
  • Whole grain, enriched bread and/or crackers
  • Dry powder milk
  • Canned evaporated milk
  • Canned ham, chicken, tuna, etc. (Less sodium in water-packed)
  • Hard natural cheeses - Cheese spreads will spoil more quickly
  • Peanut butter and nuts
  • Eggs - Keep eggs refrigerated until you are ready to use
  • Canned juices and fruits
  • Fresh fruits  (bananas, oranges, apples, etc.)
  • Dried fruits  (raisins, apricots, prunes, apples, etc.)
  • Canned vegetables - Dark green or orange vegetables preferred
  • Canned or dried soups
  • Potato flakes - Add water or milk to rehydrate

These foods require little or no cooking and should not spoil if used before their expiration date.

Home Delivered Meals - Service Criteria

Nutrition Program Home Delivered Meal Service Criteria

  • Outagamie County resident, aged 60 or older, which an assessment concludes that participation is in the individual’s best interest, and the individual is meeting home delivered meal criteria.
  • Spouse of a person eligible for a Home Delivered meal, regardless of age or condition, if an assessment concludes that participation is in the best interest of the older individual.
  • Disabled adult who resides at home with an eligible older individual participating in the Nutrition Program, if an assessment concludes that participation is in the best interest of the older individual.

Determination of Home Delivered Meal Service Need

  • Generally unable to leave home unassisted by reason of accident, illness, disability, frailty, or isolation.
  • Unable to independently obtain food and prepare adequate meals due to a lack of or inadequacy of facilities; an inability to shop, cook, or prepare meals safely; or a lack of appropriate knowledge or skill.
  • Lacks the support of family, friends, neighbors, or another meal support service in the home or community.
  • Is unable to consistently access meals at a congregate dining location due to personal health reasons or other reasons that make dining in a congregate setting inappropriate.

Conditions for Receiving Home Delivered Meal Service

  • Individual is able to feed themselves or have someone available to assist with dining.
  • Individual agrees to be home when meals are delivered or to contact the Nutrition Program when absence is unavoidable.

Family Care, Family Care Partnership or IRIS (Include, Respect, I Self-Direct) community-based long-term care members are not eligible for Home Delivered or Frozen meals unless there is a written contract, with the Outagamie County Nutrition Program and the long-term care program, for full Outagamie County Nutrition Program Home Delivered or Frozen meal cost reimbursement.  Individuals applying for Outagamie County Nutrition Program Home Delivered Meal Service will be assessed by Outagamie County Nutrition Program staff, in the individual’s home to determine the need for nutrition and other services no later than twelve (12) weeks from beginning of meal service. A formal screening for determining eligibility is required to participate. A full reassessment of continued need for Home Delivered Meal Service will be conducted annually in the individual’s home. The time period between reassessments will depend on the type of disability, the degree of isolation and the extent of other resources available to the individual.

Frozen meals and Carryout meals are considered an Outagamie County Nutrition Program Home Delivered Meal Service and must meet Home Delivered Meal Service Criteria.

Meal Cost -

The Outagamie County Nutrition Program Home Delivered meal cost is $16.47 per meal. A $5.00 suggested contribution is encouraged. Nutrition Program participants may voluntarily contribute toward the meal cost in accordance with the federal Older Americans Act. No participant will be denied meal service because they cannot or will not donate toward the meal cost. Contributions are confidential and support Outagamie County Nutrition Program Home Delivered meal service continuation.

Nutrition Program Congregate

Program Purpose

The purpose of the Nutrition Program is to reduce hunger and food insecurity, promote socialization and the health and well-being of persons aged 60 or older by providing assistance in gaining access to nutrition, disease prevention and health promotion services. The Nutrition Program provides Congregate and Home Delivered Meal Service along with nutrition education, volunteer opportunities, nutrition and benefit counseling plus current aging-related updates in accordance with the federal Older Americans Act.

Program Objectives

Prevent malnutrition and promote good health behaviors through nutrition education, screening and intervention. Serve safe and quality meals that adhere to state food safety and sanitation standards.  Promote and maintain coordination with other support services. Target older persons who have the greatest economic and/or social need with attention to low-income minority and rural older persons.

Congregate Meal Service

Congregate Meal Service provides nutritious, mid-day meals and fellowship in a communal setting. Nutrition Program Congregate meal sites offer fellowship, social and informational activities, nutrition education, health screenings, etc. An individual must meet the Congregate Meal Service criteria below and make a meal reservation in advance to eat at a Congregate meal site.

  • Individual aged 60 or older is eligible for Congregate Meal Service on a donation basis.
  • Spouse or domestic partner who attends the Congregate meal site, with their spouse or domestic partner who is aged 60 or older, is eligible for Congregate Meal Service on a donation basis. If the under aged 60 spouse or domestic partner participates, without the eligible older spouse or domestic partner, the under aged 60 spouse or domestic partner must pay the total meal cost.
  • Disabled adult, under aged 60, living in a housing facility where a Nutrition Program Congregate meal site is located, is eligible for Congregate Meal Service on a donation basis.
  • Disabled adult, under aged 60, who resides at home with an eligible older individual participating in the Nutrition Program or accompanies the eligible aged 60 or older individual to a Congregate meal site is eligible for Congregate Meal Service on a donation basis.
  • Informal caregiver (other than a spouse or domestic partner) who resides with and accompanies an eligible older individual to a Congregate meal site is eligible for meals on a donation basis.
  • Family Care, Family Care Partnership or IRIS (Include, Respect, I Self-Direct) community-based long-term care member is eligible for Congregate Meal Service on a donation basis if they meet Nutrition Program Congregate Meal Service criteria. 
  • Staff or volunteer, regardless of age, who perform Nutrition Program-related duties is eligible for Congregate Meal Service on a donation basis if it does not deprive a participant aged 60 or older of Congregate Meal Service.
  • Individual, aged 60 or older, who attends adult day care or resides in a group living home, assisted living or other residential facility is eligible for Congregate Meal Service on a donation basis if the adult day care or residential facility does not include the meal in the cost that is charged to the individual. If the individual is charged for the meal, the individual is not eligible for Congregate Meal Service unless there is a written contract, with the Outagamie County Nutrition Program and the adult day care or residential facility, for full Outagamie County Nutrition Program Congregate meal cost reimbursement.
  • Individual, under aged 60, who receives a complete Congregate meal and it does not deprive a participant aged 60 or older of Congregate Meal Service, must pay the total meal cost.

Home Delivered Meal Service

Home Delivered Meal Service provides nutritious, mid-day meals to eligible, homebound Outagamie County residents. Frozen meals are considered a Nutrition Program Home Delivered Meal Service. Home Delivered Meal Service participants are assessed and must meet the following criteria:

  • Outagamie County resident aged 60 or older, frail and essentially homebound by reason of illness, disability or isolation for which an assessment concludes that participation is in the individual’s best interest.
  • Spouse or domestic partner of a person eligible for a Home Delivered meal as described above, regardless of age or condition, if an assessment concludes that participation is in the best interest of the homebound older individual.
  • Disabled adult who resides at home with an eligible older individual participating in the Nutrition Program, if an assessment concludes that participation is in the best interest of the home bound older individual.
  • Disabled adult, under aged 60, living in a housing facility where a Nutrition Program Congregate meal site resides, if an assessment concludes that participation is in the best interest of the under aged 60 disabled adult.
  • Informal caregiver (other than a spouse), regardless of age or condition, who resides with an eligible older individual participating in the Nutrition Program is eligible to receive Home Delivered meals if an assessment concludes that it is in the best interest of the eligible older individual.

Family Care, Family Care Partnership or IRIS (Include, Respect, I Self-Direct) community-based long-term members are not eligible for Home Delivered or Frozen meals unless there is a written contract, with the Outagamie County Nutrition Program and the long-term care program, for full Outagamie County Nutrition Program Home Delivered or Frozen meal cost reimbursement.

Meal Cost

The Outagamie County Nutrition Program may not charge for meal service in accordance with the federal Older Americans Act. A suggested donation may be established. Nutrition Program participants may voluntarily donate toward the meal cost. No participant will be denied meal service because they cannot or will not donate toward the meal cost.  Donations are confidential and support Outagamie County Nutrition Program meal service continuation.

Meal Components

Nutrition Program mid-day meals meet the Recommended Dietary Reference Intakes (DRI):

  • One serving of grains
  • Three half cup servings of fruit and/or vegetable
  • Eight ounces fluid milk
  • Three ounce serving of protein
  • One teaspoon serving of fats and oils
  • One half cup serving of dessert

Outagamie County Nutrition Program meals are moderate in sodium, saturated fat and sugar. Meals are prepared, packed and delivered to preserve optimum flavor, appearance and nutritive value. All meals must conform to U.S.D.A. requirements.

Nutrition Program Funding and Program Objectives

The Nutrition Program is funded by the Federal Older Americans Act, State and local funds, and participant contributions. The Older Americans Act emphasizes regard for the dignity of older persons by requiring that opportunities are provided for older persons to participate not only in the planning and administration of the Nutrition Program but also providing older persons an opportunity to voluntarily contribute toward the cost of Nutrition Program meal service. Participant donations will be used to expand meal service, maintain service level, support access services and provide outreach, nutrition education, and nutrition counseling.
