Alzheimer's Family Caregiver Support Program (AFCSP)
The Alzheimer’s Family Caregiver Support Program (AFCSP) was created by the Wisconsin legislature in 1985 in response to the stress and service needs of families caring at home for someone with irreversible dementia. To become eligible for AFCSP funding, a person must have an Alzheimer’s disease or related dementia disorder diagnosis from their physician and live at home. AFCSP is coordinated by the Wisconsin Bureau of Aging and Disability Resources (BADR) and is available in every county throughout the state.
How does AFCSP work in Outagamie County - AFCSP funds are available through the Outagamie County Aging & Disability Resource Center (ADRC) to assist Outagamie County residents in purchasing services and goods related to the care of someone with Alzheimer’s disease or a related dementia disorder. Up to $4,000 ($333.33/month) per family annually may be available depending on a person’s need. What services and goods may be purchased with AFCSP funds? AFCSP allowable services and goods necessary to temporarily relieve a caregiver of caregiving responsibilities, support the preservation of the caregiver’s physical and mental health in caring for a person with Alzheimer’s disease or related dementia disorder in the home, and those that help maintain a person with Alzheimer’s disease or related dementia disorder as a member of the household may be purchased with AFCSP funds. Outagamie County AFCSP allowable services and goods may include in-home respite care, facility respite care, adult day care, and personal care. AFCSP service providers must have a contract with the Outagamie County Department of Health and Human Services.
Do I need a physician’s statement - The person must have an Alzheimer’s disease or other irreversible dementia diagnosis from their physician.
